By clicking on the link in the grey box at the left hand side of this page. Fill it in with your details and your charity selection(s), then pass it to your employer for deductions to be made.

This form must be printed out, filled in and then passed to your employer. It is not an online form as most employers will not accept new deductions online.

You can also use this form to amend existing payroll giving instructions.

It would be helpful if you would write AMENDMENT at the top of the form if you are just changing your deductions and have previously been giving.

Adjustments to the amount you want to donate in total must go to your employer.

Changes to your charity selections should be sent to Charities Trust, either by mail (address at the bottom of each page on this site) or by e mail to info@charitiestrust.org
